1. Promotion Description: The ‘MedPro Dental Student Box Sweepstakes' ("Sweepstakes") begins on 9/1/2021 and will be offered monthly until such time as Sponsor determines to cease the monthly Sweepstakes. By participating in the Sweepstakes, each entrant unconditionally accepts and agrees to comply with and abide by these Official Rules and the decisions of MedPro Group Inc. ("Sponsor"), which shall be final and binding in all respects. Sponsor is responsible for the collection and processing of entries and the overall administration of the Sweepstakes. Entrants should look solely to Sponsor with any questions, comments or problems related to the Sweepstakes.
2. Eligibility: Open to legal residents of USA who are 18 years or older and are currently enrolled at a dental school in the United States and are opted-in to MedPro’s Malpractice Minute Newsletter. Void where prohibited or restricted by law. Sponsor and its respective parents, subsidiaries, affiliates, distributors, retailers, sales representatives, advertising and promotion agencies and each of their respective officers, directors and employees (collectively, the "Promotion Entities"), and members of their immediate families and/or persons living in the same household as such persons, are ineligible to enter the Sweepstakes or win a prize.
3. Entry and Winner Determination: Currently enrolled dental students who are opted-in to MedPro Group’s Malpractice Minute Newsletter are entered into the monthly giveaway. Individuals can sign up on medprodental.com. Participants must be opted into the email list to be eligible can only win one box per calendar year. 100 individuals from the MedPro dental student email list will be determined as winners when they are randomly drawn by the Sponsor. The winner will be notified via the email they opted in with and will be asked to confirm their address and shirt size within 30 days to receive the box. If an individual does not respond in that time period they will not receive a box and new winner may be selected. In addition, if fewer than 100 winners confirm their address and shirt size in a given month, additional winners may be selected in the following month(s). No substitution or transfer of a prize is permitted except by Sponsor.
4. Prizes:
- 100 winners per month will be selected from the list of opted-in individuals. The selected winners will win a box full of MedPro Dental swag valued at around $30 if they respond to a winner confirmation email confirming their address and shirt size. The prize will be mailed directly to the winner. If a winner does not respond by the cutoff date, they will forfeit the prize for that month and be eligible to be randomly selected in the future.
The prizes will only be awarded and/or delivered to USA residents. All federal, state and/or local taxes, fees, and surcharges are the sole responsibility of the prize winner. Failure to comply with the Official Rules will result in forfeiture of the prize.
5. Publicity Release: Except where prohibited by law, in consideration for being permitted to participate in this Sweepstakes and the opportunity to win the prize offered in this Sweepstakes, each entrant hereby agrees and consents, without further authorization, compensation or remuneration of any kind, to the use of the entrant’s name, likeness, voice, photograph, performance, personal statements and/or biographical information in any and all advertising, promotions and other publicity conducted by Sponsor and each of their respective parents, subsidiaries, affiliates, agents, licensees, successors and assigns.
6. Privacy: All entrants acknowledge that if they are chosen as a winner, certain of their personally identifying information may be disclosed to third parties at least as required by law, including, without limitation, on a winner's list. All personal information submitted as part of the Sweepstakes may be shared by and between the Promotion Entities, and each of the Promotion Entities will use such information in accordance with their respective independent online privacy policies. 7. Limitation of Liability: Sponsor is not responsible for any incorrect or inaccurate information which may occur in the processing of submissions in the Sweepstakes. Sponsor assumes no responsibility for any misdirected or lost mail, or any error, omission, interruption, deletion, defect, delay of operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. If, for any reason, the Sweepstakes is not capable of running as planned, including, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Sweepstakes, then Sponsor reserves the right in its sole discretion to cancel, terminate, modify or suspend the Sweepstakes. By entering, each entrant agrees to follow these Official Rules and forever releases, discharges, and waives any and all claims, liabilities and actions of any kind, howsoever caused or arising and whether by negligence, any intentional act or otherwise, that entrant may have or that may arise against Sponsor and its respective parents, subsidiaries, affiliates and related entities, as well as each of the aforementioned parties’ respective directors, officers, owners, shareholders, members, employees, representatives, successors, assigns and agents (collectively, the “Released Parties”), with respect to or arising out of either (i) entrant’s participation; or (ii) the receipt or use of the prizes awarded in the sweepstakes. By entering, each entrant further agrees to defend, indemnify and hold harmless the Released Parties and their insurers against any and all damages, costs, judgments, and/or expenses (including but not limited to attorneys’ fees and/or other legal fees) they may incur as a result of claims made against them in connection with either (i) entrant’s participation; or (ii) the receipt or use of the prizes awarded in the sweepstakes.
8. Winner's List: To obtain a copy of any legally required winners list or a copy of the Official Rules, send the applicable request and a self-addressed, stamped, #10 envelope to MedPro Group Inc. cc: MedPro Group Dental Box at 5814 Reed Rd, Ft Wayne IN 46835, Attn: Emily Hetrick. Requests must be received no later than 6 MONTHS after the close of the Sweepstakes.
